Notes: Despite a roughly 25% premium increase in 2026, and Max Out of Pockets up to $12,000 for 2027, the advanced premium tax credits can make ACA plans very affordable. ACA plans are ideal for people with a predictable income between 150% and 400% of federal poverty level (FPL). Its important to check your doctors as most ACA plans are now HMO and may not be widely accepted by providers. For people who earn more than 400% of FPL the ACA has been dubbed the 'unaffordable care act'. Premiums are particularly high after age 50.

Notes: In 2027 Justin is contracted with four different companies that offer health insurance as a benefit of work from home employment. Some companies require you to fill out health related surveys (and yes you get paid) while others require you to use their App. Some pay you as a W-2 employee, while other pay you as a K-1 working owner. Rates can be considerably lower than ACA and benefits can be stronger. Major Medical plans include a true maximum out of pocket (just like the ACA). Unlike most ACA plans, these plans use nationwide PPO networks.

Notes: In 2027 Justin is contracted with TWO different companies that offer Visit Limit plans as benefit of becoming a W-2 work from home employee. Unlike the ACA, nationwide PPO plans are often available.

*- Visit limit plans are subject to a certain number of visit limits, and therefore do not have a true Maximum out of Pocket. However they do have excellent wellness and preventative benefits. The premiums are also more affordable than major medical.

Provider Lookup Tool: Most carriers use the First Health network to get network negotiated rates, but its a PPO so you can go anywhere. United Healthcare uses the united network to provide indemnity coverage thru UH One, but their benefits are generally not as rich as other carriers.

Notes: Justin works with 4 major carriers that offer bundled indemnity insurance options. These plans are highly customizable. There is no deductible - because they provide FIRST DOLLAR Coverage and use a Nationwide PPO Network. You can go anywhere, but you'll get network negotiated rates by using the First Health network. Indemnity plans are meant to be bundled. Add Accident, Critical Illness, and Specified Disease coverage to protect you from worst case scenarios. Notes: Short term medical is good for those on a budget who need catastrophic coverage. It is often used to bridge a gap, such as while waiting for the ACA open enrollment period or waiting for employer coverage to begin. These plans are simplified underwriting but in general they DO NOT COVER pre-existing conditions. There is often a 90 day wait after which time certain preventative services are covered with no copay. Each state has different laws governing short term medical insurance. In Michigan short term medical plans can only be 6 months duration, but most other states can get 1 to 3 years of short term coverage. Conditions that develop while you are covered are not considered pre-existing conditions and wil be covered within certain limits.

In most states this plan includes a $200 annual wellness benefit that you can use once per year however you choose. This is the most comprehensive specified disease plan that I have seen on the market and includes the following specified diseases: Addison’s Disease, Cancer, Coronary Artery/Cardiovascular Disease, COVID-19, Cushing’s Disease, Diabetes, Ebola Virus Disease, End Stage Renal Failure, Gallbladder Disease, Gastrointestinal Disease, Gout, Hyperthyroidism, Hypothyroidism, Major Organ Transplant, Pancreatitis, Pneumonia, Psoriasis, Shingles, Stroke, Viral Hepatitis

After the $250 deductible, it pays a cash benefit for accidents, heart attack, stroke, major organ transplant, coronary bypass surgery, heart valve replacement, invasive cancer, cancer in situ, end stage renal disease, advanced Alzheimer's disease, coma, motor neuron disease (ALS), paralysis, or severe burns. Can be added to a major medical plan to offset high deductibles.
